Interview form?

Hi, I have all my tests and interview in 2 days for Primary Reserve Infantry. I was going through the forums for interview prep and everything and I saw something about an interview form you must fill out before you go? They were some old posts back from '08 but I'm now curious to know if theres any forms I need to fill out before my interview because I wasn't told about any form. Also it's proper to wear a clean dress shirt with a tie for the interview correct?

Sorry just one more question, it might be a bit silly, but for the reserves are you required to write up a resume and bring it to the interview? Or do all the forms you filled out count.
 
Kyle, I brought a portfolio for my reserve interview. They looked at it, but it wasn't necessary in the end (in my case). Besides, I felt more confident knowing I was prepared rather than walking in with nothing.

Forgive me for assuming, but based on the way you posed your question, I urge you to write up your resume not just for the CF interview, but for yourself. It's best to start sooner than later to track your education, experiences, accomplishments, etc.
