I've misplaced my blue cross card. How should I go about getting it voided and replaced? Do I have to go through my chain of command, or Blue Cross separately?
Your number is easy to remember; it is your Service Number with the first letter being changed. Your Service Number will be listed above it as well; as will your DOB. You will probably be getting a new card in a couple of years anyway. Visit your OR and perhaps speed up that process.
